HOW TO APPLY FOR SOLO PARENT ID | Qualifications, Requirements and Benefits


 A solo parent identification card is a government-issued identification card that solo parents can use to avail of certain government discounts and benefits.

 

Qualifications to Get the Solo Parent ID


A solo parent, as defined by RA 8972 is:

1. A woman who gives birth as a result of rape and other crimes against chastity even without a final conviction of the offender, provided that mother keeps and raises the child.

2. Parent left solo or alone with the responsibility of parenthood due to the following circumstances:

a. Due to death of spouse.

b. Spouse is detained or is serving sentence for a criminal conviction for at least one (1) year.

c. Physical and/or mental incapacity of spouse as certified by a public medical practitioner.

d. Legal separation or de facto separation from spouse for at least one (1) year, as long as he/she is entrusted with the custody of the children.

e. Declaration of nullity or annulment of marriage as decreed by a court or by a church as long as he/she is entrusted with the custody of the children.


3. Unmarried mother/father who has preferred to keep and rear her/his child/children instead of having others care for them or give them up to a welfare institution.
 

4. Any other person who solely provides parental care and support to a child or children.

5. Any family member who assumes the responsibility of head of family as a result of the death, abandonment, disappearance or prolonged absence of the parents or solo parent.



Criteria for Support


Any solo parent whose income falls below the poverty threshold as set by the National Economic and Development Authority (NEDA) and subject to the assessment of the DSWD worker in the area shall be eligible for assistance. A solo parent can directly inquire from the following agencies to avail of their services:

  • Health Services (DOH)
  • Educational Services (CHED, TESDA)
  • Housing (NHA)
  • Parental Leave (Employer, DOLE, CSC) Note: A solo parent whose income is above the poverty threshold shall enjoy only such limited benefits as flexible work schedule, parental leave and others to be determined by the DSWD.


Requirements

 

  • Application Form obtainable from the local office of the Department of Social Welfare and Development or through download.
  • Barangay certification certifying Solo Parent’s residency in the barangay for the last six months
  • Certificates e.g., birth certificates of children, death certificate of spouse and other appropriate documentary support
  • Income tax return or any document that will establish the income level of the solo parent
  • 1x1 ID Picture
 
Additional Requirements
 
Additional requirements needed depending on the kind of solo parents:
 
1. For a solo parent with child or children as consequence of rape
  • birth certificate
  • complaint affidavit
  • medical record on the incident of rape
  • sworn affidavit declaring that the solo parent has sole parental care of his or her child or children
2. For a solo parent with a deceased spouse
  • birth certificate/s of child or children
  • marriage certificate
  • death certificate of the spouse
  • sworn affidavit declaring that the solo parent is not cohabiting with a partner or a co-parent and has sole parental care of the child or children
3. For a solo parent whose spouse is under detention or is convicted of a crime
  • birth certificate/s of child or children
  • marriage certificate
  • certificate of detention of the spouse certifying that he or she is serving sentence of at least three months or a commitment order pursuant to a conviction
  • sworn affidavit declaring that the solo parent is not cohabiting with a partner or a co-parent and has sole parental care of the child or children
4. For a solo parent whose spouse becomes physically or mentally incapacitated
  • birth certificate/s of child or children
  • marriage certificate or affidavit of cohabitation
  • medical record or abstract evidencing the physical or mental state of the spouse issued more than three months prior to application
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children
5. For a solo parent under legal or de facto separation of spouse and for a solo parent whose marriage is no longer valid due to annulment or divorce
  • birth certificate/s of child or children
  • marriage certificate
  • judicial decree of legal separation of spouses; in case of de facto separation, an affidavit of two disinterested persons attesting the separation; while for annulment or divorce cases, judicial decree of nullity
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children
6. For a solo parent abandoned by spouse
  • birth certificate/s of child or children
  • marriage certificate or affidavit of the applicant solo parent
  • affidavit of two disinterested persons attesting to the abandonment of spouse
  • police or barangay record of the abandonment
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children
7. For a solo parent or any family member of a low or semi-skilled overseas Filipino worker deployed continuously abroad for at least a year
  • birth certificate/s of child or children
  • marriage certificate of applicant
  • overseas employment certificate or any equivalent document
  • copy of passport stamps showing continuous 12 months of overseas work
  • employment contract
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children
8. For an unmarried father or mother keeping and rearing child
  • birth certificate/s of child or children
  • certificate of no marriage or CENOMAR
  • affidavit of a barangay official attesting the residence of the solo parent and his or her solo parental care of the child or children
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children
9. For a solo parent who is a legal guardian or adoptive/foster parent
  • birth certificate/s of child or children
  • proof of guardianship or adoption
  • affidavit of a barangay official attesting the residence of the solo parent and his or her solo parental care of the child or children
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children
10. For any relative within 4th consanguinity or affinity of the parent who assumes parental guidance
  • birth certificate/s of child or children
  • death certificate of parents or police/barangay records evidencing the disappearance or absence of the parent/s for at least six months
  • affidavit of a barangay official attesting that the child/children is/are under parental care of the applicant
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children
11. For a pregnant solo parent
  • medical record of pregnancy
  • affidavit of a barangay official attesting that the applicant solo parent is a resident and has no spouse
  • sworn affidavit declaring that the solo parent is not cohabiting with a parent or co-parent and has sole parental care of the child or children

Meanwhile, for solo parents who will avail themselves of subsidies and discounts provided under Section 15 of the law, the following must be submitted:
  • affidavit of no employment
  • income tax return
  • social case study issued by the Department of Social Welfare and Development, or
  • any verifiable proof of income

How to Apply


1. Head to your Department of Social Welfare and Development office in your city or municipality to apply for a Solo Parent ID. 

2. Fill out the application form and submit it together with the documents mentioned above.

3. Pay the required fees.

4. Wait for your application to be approved. The ID will be issued after 30 days from filing.

The social worker receives and ensures that all documents are complete and registers the applications with an appropriate case number in the log-book Registry of Solo Parents.

The validity of the ID is one year and is renewable.
